- Evidence of COVID-19 Vaccination
- Good Conduct Certificate

- Completion of Required Documents by Hospital Wilma Vazquez
- Completion of the “Application Shadow” Document by Your Primary Physician
Application for Shadowing Experiences
Requirements:
- Brief declaration of interest
- Copy of the class program
- Evidence of vaccination against COVID-19 or vaccine exemption for health reasons
- ID with photo (driver’s license or passport)
- CV
The accepted student will be asked for a donation of $10 to cover student liability insurance and operational expenses.
